Nominate Someone Today

The Nomination Form is a PDF Fill-In Form.

1. Download the form and save it to your computer (save it with the nominee’s name in the filename).
2. Complete the form electronically by typing the information.
3. Save the form again.
4. Attach it to a new email.
5. Email the PDF file to the Nominating Committee Chair.
6. If you decide to print it after you electronically complete it, you can mail it to the Nominating Committee Chair.
7. You can also download it, print it, and fill it in by hand if you would like to do so.
